There is a trick to do this without having to do three playthroughs. This is what I did. I stayed as good karma for my status. When I got to the rank below the rank that is required for the trophy I would create a second save file and go to a town and kill the people in that town until I was neutral then level up. Then I would reload my file and repeat until I was evil. Then reload my primary file and continue my game as good karma.
